How ERG Reflective handles your personal information when you contact the practice, book an appointment or receive a service.
Booking information includes your name, email address, phone number, chosen service, appointment date and time, and meeting format. The practice may also hold correspondence, attendance records, clinical notes, documents you provide, payment records and receipts.
Information is collected from you when you book, contact the practice or attend a session, and from records created by the practitioner while providing your care. If information is needed from another person or provider, the practice will discuss consent with you where required.
Please keep the booking form to contact and scheduling details. It is not intended for medical histories or clinical documents.
Your information is used to arrange and provide services, maintain care records, communicate with you, manage payments and receipts, and meet applicable record-keeping obligations. Authorised practice staff can access information according to their role.
Your contact details are used for confirmations, reminders and appointment updates. Appointment emails include booking details and may include a private management link or Zoom joining link. Keep these links private, particularly when using a shared inbox.
The website and portal use Google Firebase services for hosting, staff sign-in, database records and file storage. Staff access requires an authorised account, and permissions restrict access to client records and files.
An email delivery provider processes appointment messages and emailed receipts. Zoom is used to arrange and deliver video appointments. These providers process information needed to supply their services under their own terms and privacy policies. The portal does not enable automatic Zoom recording when it creates a meeting.
Service providers may process information outside Australia. The countries involved depend on the provider and the practice’s account settings. Contact the practice for information about the providers and overseas handling that apply to your records.
Contact the practice to request access to your personal information or to correct details that are inaccurate or out of date. We may need to verify your identity before providing information or making changes. If access or a correction cannot be provided, we will explain the reason and the options available to you.
You can ask the practice about deleting your information. Requests are reviewed against applicable health-record, financial and other legal retention requirements. Some records may need to be kept even after your appointments end or you request deletion.
Deleting a client profile from the portal removes linked client and appointment records, notes and uploaded files. Limited technical records of the deletion are retained. Portal deletion does not recall emails already sent or remove copies already downloaded or held separately by recipients or service providers. Those copies require separate handling.
Contact the practice using the details below and describe your question or concern. We will review the matter, contact you if more information is needed and explain the outcome. Please avoid including sensitive clinical details in an initial email.
If your concern is not resolved, you can contact the Office of the Australian Information Commissioner for information about making a privacy complaint.
